- ItemDevelopment and evaluation of a PDA-based decision support system for pediatric depression screening.(2007-10-03) John, Ritamarie; Buschman, Penélope; Chaszar, Mosely; Honig, Judy; Mendonca, Eneida; Bakken, SuzanneDepression is under recognized in a variety of pediatric settings The purpose of this paper is to describe the development and initial evaluation of a personal digital assistant PDA based decision support system DSS for pediatric depression screening in ages 8 to 18 years of age by pediatric advanced practice nurse APN students Three aspects are described selection of depression screening instrument integration of the instrument into the PDA and quantitative usage and qualitative focus group evaluation Only one third of eligible patients were screened Twenty percent of those screened were identified as at risk for mood disorder The barriers to screening identified through focus groups included a lack of time knowledge intervention protocol referral resources PDA usability issues preceptor motivation and comfort as well as perceived or real cultural barriers Suggestions for educational research and interventions to integrate clinical based PDA based screening are discussed
- ItemDevelopment of a prototype personal digital assistant-decision support system for the management of adult obesity.(2007-09-03) Lee, Nam-Ju; Bakken, SuzanneOBJECTIVE The purpose of this study was to develop a prototype personal digital assistant decision support system PDA DSS based on a clinical practice guideline CPG for the management of obesity DESIGN The study was composed of four phases 1 analysis of advanced practice nurse APN students documentation related to the management of obesity using the Clinical Log APN CL APN 2 identification of functional requirements and data modeling through use case analysis and unified modeling language UML 3 evaluation of representation of obesity related concepts with standardized terminologies and 4 design of a web based prototype user interface RESULTS The analysis revealed the documentation rate of obesity as an assessment diagnosis and adherence to the CPG for obesity was low Through use case analysis and UML modeling the functional requirements screening assessment and documentation of CPG based obesity treatment plan were identified and a data model was built Overall the standardized terminologies that are used in the database for the CL APN could represent about 80 of the obesity related concepts However the terms of these standardized terminologies were not specific enough to represent all the concepts The systematized nomenclature of medicine clinical terms SNOMED CT could represent 83 of the concepts and was used to extend the knowledge base Based on the functional requirements specification four prototype screens were designed CONCLUSION The PDA DSS for the management of obesity has potential uses for education nursing practice and research As an educational tool it can be used to improve APN students adherence to the CPG s recommendations and to enhance informatics competencies The PDA DSS has the potential to improve APN students clinical decision making at the point of care and delivery of CPG based care thereby improving patients outcomes related to the management of obesity A randomized trial is underway to investigate the PDA DSS s impact on APN students screening rates and adherence to obesity CPG recommendations and users acceptance of the system

- ItemThe effect of a mobile clinical decision support system on the diagnosis of obesity and overweight in acute and primary care encounters.(2009-08-26) Lee, Nam-Ju; Chen, Elizabeth S; Currie, Leanne M; Donovan, Mary; Hall, Elizabeth K; Jia, Haomiao; John, Rita Marie; Bakken, SuzanneThe purpose of the study was to compare the proportion of obesity related diagnoses in clinical encounters N 1874 documented by nurses using a personal digital assistant based log with and without obesity decision support features The experimental group encounters in the randomized controlled trial had significantly more P 000 obesity related diagnoses 11 3 than did the control group encounters 1 and a significantly lower false negative rate 24 5 vs 66 5 P 000 The study findings provide evidence that integration of a decision support feature that automatically calculates an obesity related diagnosis increases diagnoses and decreases missed diagnoses and suggest that such systems have the potential to improve the quality of obesity related care

- ItemHealth literacy screening instruments for eHealth applications: a systematic review.(2012-06-08) Collins, Sarah A; Currie, Leanne M; Bakken, Suzanne; Vawdrey, David K; Stone, Patricia WOBJECTIVE To systematically review current health literacy HL instruments for use in consumer facing and mobile health information technology screening and evaluation tools DESIGN The databases PubMed OVID Google Scholar Cochrane Library and Science Citation Index were searched for health literacy assessment instruments using the terms health literacy computer based and psychometrics All instruments identified by this method were critically appraised according to their reported psychometric properties and clinical feasibility RESULTS Eleven different health literacy instruments were found Screening questions such as asking a patient about his her need for assistance in navigating health information were evaluated in seven different studies and are promising for use as a valid reliable and feasible computer based approach to identify patients that struggle with low health literacy However there was a lack of consistency in the types of screening questions proposed There is also a lack of information regarding the psychometric properties of computer based health literacy instruments LIMITATIONS Only English language health literacy assessment instruments were reviewed and analyzed CONCLUSIONS Current health literacy screening tools demonstrate varying benefits depending on the context of their use In many cases it seems that a single screening question may be a reliable valid and feasible means for establishing health literacy A combination of screening questions that assess health literacy and technological literacy may enable tailoring eHealth applications to user needs Further research should determine the best screening question s and the best synthesis of various instruments content and methodologies for computer based health literacy screening and assessment
- ItemInformatics competencies pre-and post-implementation of a Palm-based student clinical log and informatics for evidence-based practice curriculum.(2004-01-19) Bakken, Suzanne; Sheets Cook, Sarah; Curtis, Lesly; Soupios, Michael; Curran, ChristineThe purpose of this paper is to describe the implementation and evaluation of a two part approach to achieving informatics competencies 1 Palm based student clinical log for documentation of patient encounters and 2 informatics for evidence based practice curriculum Using a repeated measures non equivalent control group design self reported informatics competencies were rated using a survey instrument based upon published informatics competencies for beginning nurses For the class of 2002 scores increased significantly in all competencies from admission to graduation Using a minimum score of 3 on a scale of 1 not competent and 5 expert to indicate competence the only area in which it was not achieved was Computer Skills Education For 2001 graduates Computer Skills Decision Support was also below 3 There were no significant differences in competency scores between 2001 and 2002 graduates Computer Skills Decision Support neared significance Subsequently the approaches were refined for implementation in the class of 2003
- ItemmHealth Technology as a Persuasive Tool for Treatment, Care and Management of Persons Living with HIV.(2015-07-09) Schnall, Rebecca; Bakken, Suzanne; Rojas, Marlene; Travers, Jasmine; Carballo-Dieguez, AlexMobile health mHealth technology can be a valuable tool in the management of chronic illnesses including HIV Qualitative research methods were used to identify the desired content and features of a mobile app for meeting and improving the healthcare needs of persons living with HIV PLWH We conducted six focus group sessions with 50 English or Spanish speaking PLWH in New York City To inform data analysis and to illustrate how mHealth technology can be used as a persuasive strategy for improving the health of PLWH we integrated Fogg s functional role triad for computing technology model with the self determination theory to illustrate how mHealth technology can be used as a persuasive strategy for improving the health of PLWH Participants suggested several tools for meeting their healthcare needs including reminders alerts lab results tracking and notes on health status mHealth technology can function as a social actor by providing chat boxes forums testimonials of lived experiences and personal outreach Examples of media that can be used as a persuasive technology include games virtual rewards coding of health tasks and simulations on how to connect with PLWH Findings from these focus groups can be used to design a mobile app for PLWH that is targeted to meet their healthcare needs

- ItemMobile health-based approaches for smoking cessation resources.(2013-06-27) Hyun, Sookyung; Hodorowski, Julie Keany; Nirenberg, Anita; Perocchia, Rosemarie Slevin; Staats, Jo Anne; Velez, Olivia; Bakken, SuzannePURPOSE OBJECTIVES To describe how the National Cancer Institute s Cancer Information Service CIS smoking related resources on a mobile health mHealth platform were integrated into the workflow of RNs in advanced practice nurse APN training and to examine awareness and use of CIS resources and nurses perceptions of the usefulness of those CIS resources DESIGN Descriptive analyses SETTING Acute and primary care sites affiliated with the School of Nursing at Columbia University SAMPLE 156 RNs enrolled in APN training METHODS The integration was comprised of a inclusion of CIS information into mHealth decision support system DSS plan of care b addition of infobutton in the mHealth DSS c Web based information portal for smoking cessation accessible via desktop and the mHealth DSS and d information prescriptions for patient referral MAIN RESEARCH VARIABLES Use and perceived usefulness of the CIS resources FINDINGS 86 of nurses used the mHealth DSS with integrated CIS resources Of the 145 care plan items chosen 122 were referrals to CIS resources infobutton was used 1 571 times Use of CIS resources by smokers and healthcare providers in the metropolitan area of New York City increased during the study period compared to the prestudy period More than 60 of nurses perceived CIS resources as useful or somewhat useful CONCLUSIONS Integration of CIS resources into an mHealth DSS was seen as useful by most participants IMPLICATIONS FOR NURSING Implementation of evidence into workflow using an mHealth DSS can assist nurses in managing smoking cessation in patients and may expand their roles in referring smokers to reliable sources of information KNOWLEDGE TRANSLATION mHealth DSS and information prescriptions may support smoking cessation interventions in primary care settings Smoking cessation interventions can be facilitated through informatics methods and mHealth platforms Nurses referrals of patients to smoking related CIS resources may result in patients use of the resources and subsequent smoking cessation

- ItemPreliminary analysis for the development of a PDA-based decision support system for the screening and management of obesity.(2006-11-14) Lee, Nam-Ju; Bakken, SuzanneThis preliminary study toward the development of a Mobile Decision Support System for Advanced Practice Nursing MODS APN for the management of obesity compared APN students current electronic clinical log CL APN documentation to an obesity clinical practice guideline CPG and examined the extent to which concepts related to the screening and management of obesity could be represented with standardized terminologies The data that were documented by APN students with the CL APN were retrieved using the Structured Query Language SQL and were compared to the CPG using a scoring tool developed for this study Only 1 4 of the encounters had obesity as an assessment diagnosis and about 40 of those encounters did not have any plans of care related to the CPG s main treatment recommendations Fifty six concepts related to the screening and management of obesity were extracted from the CPG and were mapped to 36 terms using standardized terminologies
